Administering Medicines
If your child needs to be given medicine within the school day, please print and complete the form below and take it to the school office along with the medicine that your child requires.
All medicines will be kept securely within the office and must be collected by an adult at the end of the school day. Please note that medication will not be administered to any child unless this form is completed and returned to the school office.
